2/12/2024 EnchantaVaughn Approved QMHPA greeted Quaaniensha upon arrival to the home setting. Upon arrival to the home setting, Quaaniensha appeared to be in a positive mood. QMHPA inquired about her day. Quaaniensha verbalized to QMHPA that she was having an okay day. QMHPA informed Quaaniensha what this session was going to focus on. Quaaniensha did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Quaanienshaâs current thoughts, feelings and emotions. QMHPA educated Quaaniensha on effective communication skills. QMHPA engaged Quaaniensha in a skills building activity. QMHPA informed Quaaniensha that the purpose of this session was for QMHPA to demonstrate to Quaaniensha appropriate communication skills to apply when conversing. QMHPA discussed with Quaaniensha improving her ability to express her thoughts through appropriate verbalization. QMHPA conducted another skills building activity. QMHPA explained to Quaaniensha that the purpose of this activity was to assist her with identifying positive self-descriptive statements. QMHPA processed with Quaaniensha about participating in enjoyable activities to reduce depressive symptoms. QMHPA discussed with Quaaniensha trust issues and abandonment issues. QMHPA explained to Quaaniensha the importance of being able to endure in enjoyable activities to avoid depression and anxiety. Quaaniensha appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. During the skills building activity, Quaaniensha was agreeable with learning effective communication skills. Quaaniensha struggles with effective communicating. Quaaniensha said she does not like to share her thoughts and feelings with others. Quaaniensha was able to identify positive things about herself. Quaaniensha said she is happy, she loves her family, but she tries to find it deep within her soul to be able to get along with her daughter and not to get depressed when she calls or texts her.
